To Enroll: Complete the form on the right with all the medications requiring refill reminders and dose alerts.  Choose whether text or email notifications are requested.  Choose the preferred method of delivery; TEXT, EMAIL, & PHONE CALLS are options.  Be sure to include the medication name(s), the times you take the medication so that you are notified at the correct time of day.  Also be sure to include a stop date if the medication is a temporary drug.  If you would like to get both refill reminders and dose alerts, submit one then refresh the screen and resubmit the other.  One of our staff members will contact you to confirm enrollment and the accuracy of the information submitted.
